SmartPush’s Embedded Forms help you collect customer information directly from your store pages. Once you’ve created a form in SmartPush, you can insert it into key parts of your SHOPLINE store, such as the homepage, thank you page, or custom pages.
This article walks you through three common usage scenarios and how to embed SmartPush forms accordingly.

Prerequisite: Setting Up an Embedded Form in SmartPush
Before embedding your form, follow the steps in Setting up Embedded Forms to:
- Create your form.
- Customize questions, styles, and required fields.
- Save and copy the embed code.
Once the form is activated, the embed code will also be available under the Embed code section.
Embedding the form on the Homepage (via Custom HTML)
Recommended for: Welcoming first-time visitors and encouraging them to subscribe for updates or discounts.
Steps:
- Go to SHOPLINE Admin > Online Store and click Design.
- Click Add component > choose Custom HTML.
- Paste the SmartPush embed code into the HTML box.
- Save and publish your changes — your form will appear on your homepage.
Embedding the form on a Custom Page (via Custom HTML)
Recommended for: Creating a standalone form page or combining it with blog, FAQ, or landing content.
Steps:
- In SHOPLINE Admin > Online Store > Page, create or edit a custom page.
- Click Add component > Custom HTML.
- Paste the SmartPush embed code and save.
Embedding the form on the Thank You Page (via Custom Code App)
Recommended for: Collecting additional info right after a customer completes a purchase.
Steps:
- In SHOPLINE Admin, go to the Custom Code app.
- On the app's homepage, click Add code.
- Choose the trigger: Checkout (This trigger includes Thank You Page).
- Paste the SmartPush embed code in the Code content field.
- Click Update, and your form will now show up on the order confirmation (Thank You) page.

Accessing Collected Customer Information
Once users fill out the form:
- Go to SmartPush > Contacts > Segments.
- View submissions and use collected fields (like preferences, answers) to segment your audience.
- You can apply these segments in automated flows and email/SMS campaigns.
